Adult Day Programs
Adult day care health services are designed for younger or older adults who have a chronic physical or cognitive impairments. Those who can benefit from the services offered by adult day care include: adults with Alzheimer’s or other forms of dementias, Parkinson’s, neuromuscular diseases or developmental disabilities; victims of stroke, heart attack or traumatic brain injury; elderly adults who are homebound and in need of social stimulation; and those who require assistance with daily activities such as eating, toileting and bathing.

Active Day / Senior Care adult day care health centers partner with caregivers, case managers and physicians in a team approach to provide each member with a unique program of cognitive and physical activities that is custom-tailored to his or her needs and interests. By engaging in these activities, our members receive mental and physical stimulation that helps to improve their well-being and quality of life.
Adult Day Health Services: Medication Administration, Transportation, Nutritious Meals and Snacks, Physical Therapy, Occupational Therapy, Speech Therapy, Social Services, Therapeutic Recreational Activities, Outings, and Pet Therapy.
