    Hospice at Home

    Hospice patients may require different intensities of care during the course of their disease. The Medicare Hospice Benefit affords patients four levels of care to meet their clinical needs, these levels are: routine home care, continuous home care, general inpatient care, and inpatient respite care. The Hospice Benefit was created to ensure that care and services are offered where the patient lives, whether that is at home, assisted living, nursing home, or hospice house.

    Important components of hospice care services are palliative or supportive care. This type of care helps provide a better quality of life for the patient in the final stages of their illness. The Agapé Hospice team is able to offer relief for the patient by taking a comfort-oriented approach to pain and symptom management. Our hospice staff knows how to provide crisis care and palliative services for those in the advanced stages of their illness.

    Hospice Services are provided 24 hours a day, 365 days a year

    Registered Nurse will review history and complete an evaluation assessment for qualification of the Hospice benefit.
