Palliative Care of South Carolina was created to provide specialized medical care for people with serious illness. Our goal is to improve the quality of life for both the patient and the family. We focus on providing relief from the symptoms and stress of advance illness by providing a team of specialists that work alongside the patient’s physicians.

With palliative care, patients will receive physical, emotional and spiritual support. The team helps to relieve pain and symptoms- such as fatigue, anxiety, shortness of breath, nausea, and depression. They also will help the patient decide the next steps; the plan and goals of care. Palliative Care of South Carolina is here to help at any stage of serious illness, regardless of age or diagnosis.

Resources can be found in our printed magazines and through our online directory.  Both items also include articles and other important information for seniors and caregivers.

Provides patients with expert care for pain control and systems management, offers caregivers the opportunity for respite, and furnishes a safe place for patients who may need inpatient hospice care. Hospice & Community Care also offers grief support and education for caregivers, families, and community members. A Veterans program provides services and resources to our veteran patients and their families.

We are a Move Management Company, specializing in helping older adults and their families manage the often overwhelming process of downsizing and moving.

We offer a wide variety of activities to enhance the quality of life for our participants and their families. We serve both the senior population and young adults with developmental disabilities, individualizing programs to assist them to continue to grow, learn and contribute to the society. A hot lunch is served daily. 

We also offer free trial day passes at our day centers for those who would like to try it out, and accept many payer sources, such as Medicaid and VA. 

The Greenville Area Parkinson Society (GAPS) is a local non-profit organization in the Upstate of South Carolina that offers support, education, and advocacy to people living with Parkinson's Disease, their care partners, spouses and families.

HopeHealth began providing primary care services at Bethea Retirement Community in June 2017. This partnership allows HopeHealth to provide convenient care to residents, helping patients maintain their independence.

With services open to the general public as well, patients are not required to be a resident of Bethea to receive care at HopeHeath at Bethea. HopeHealth at Bethea is now recognized as a level 2 Age-friendly Health System.

Certified in 2012, Pathway Hospice was born out of the vision of a family owned company who realized the value of hospice care and wanted it for their families and neighbors.

Today, their vision and mission remain the cornerstones of our work and organizational soul.

Pathway Hospice is committed to serving those living with terminal illnesses, nearing the end of life, or experiencing grief by caring for their patients and helping to guide their families as they provide care and make decisions.

Pathway Hospice is community based, offering comprehensive hospice and palliative care, caregiver education, individual and group counseling, spiritual support and many other programs to those it serves regardless of race, age, faith, diagnosis or financial circumstances.

Continuing Care Retirement Community - Provide all levels of senior housing from Independent Living (Cottages, Patio Homes and Garden Apartments), Assisted Living, Rehab, Memory Care and Long Term Care Nursing Center.

The Legacy of North Myrtle Beach is a senior living community offering assisted living apartments, bridge to rediscovery program for seniors with Alzheimer's or related dementias, and respite care (short stays).

Vital Aging of Williamsburg County, Inc. provides aging services for those in need.

Palmetto Family Homecare is faith-based independent provider servicing the state of South Carolina.  Palmetto Family Homecare is a licensed DHEC provider, providing non-medical care in the homes of clients and within facilities.  We have a family oriented approach to our care, while specializing in Neurological Disorder and Autism Care.  We provide an array of services to the elderly, disabled community, veterans, rehab recovery clients, new mothers or just anyone in the need of an extra pair of hands.

The Brain Injury Association of South Carolina is a nonprofit organization which was developed by persons with brain injury, their families, and concerned professionals in an effort to provide information and support to those who have experienced brain injury.

The BIASC is our state’s chartered affiliate of the national Brain Injury Association of America. Our mission is to create a better future through brain injury prevention, research, education and advocacy.

FCCOA is a nonprofit based in Winnsboro, SC, that collaborates with organizations in Fairfield County on providing senior services. FCCOA receives funding and guidance from federal, state, local governments and generous donors to carry out its mission of providing senior services and resources to improve the quality of life for the county's senior population.

Vision - Fairfield County Council on Aging aims to address the physical, social, mental and emotional needs of the elder population by equipping them with knowledge, assisting them with nutritional requirements and providing access to programs and services designed to elevate their well-being.

Channel the Beacon, Inc. provides Care Coordination/Case Management for individuals with disabilities and special needs.

DD&C is a dedicated company based on providing care for the ill and the disabled. Their hardworking staff will go above and beyond to make sure that your family member is taken care of.

The Carolinas Center (TCC) is the association for the hospice and end of life industry throughout North & South Carolina. Since its inception in 1977, TCC has been a powerful advocate and leading voice for the industry. TCC's extensive educational offerings, technical assistance and discipline-specific newsletters are known nationally for their excellence.

We are the only full placement service that can assist with Medicaid placements for assisted living and skilled care!

Full Service Placement Assistance - Whether you are thinking about assisted living, dementia care, independent senior living or skilled nursing, we can help you identify the right level of care and find the best community that fits the needs of your loved one.  This is a no cost service to you.  It's about what matters most to you and your family, current and future care, personal preferences, location and budget.  We arrange the visits at your chosen communities, personally accompany you on tours, and coach you to ask the right questions.  We track the communities compliance with state regulations.  You don't have to do this alone!

Elder Care Consulting - There is so much going on.  You are trying to balance family, work, and supporting a loved one.  We can assist with cutting through the never-ending options, anecdotes, and advice.  We help you sort through all of it, create a realistic step-by-step plan to navigate the many decisions you will face, and help you access appropriate, vetted resources.  Bringing in home care, putting a plan of care in place so you aren't dealing in a crisis.  Need someone to be your loved ones care advocate as the adult children are out of town or out of state?  We can be that extension and extra hands and eyes for your loved one while they are in the hospital or rehab.

Greater Greenwood United Ministry, established in 1995, is a 501c3 non-profit organization that assists individuals in the community with spiritual, physical, and emotional needs. With the help of a partnership with Christian churches, business, and industries, the Ministry helps individuals move beyond their life or medical crisis to make better life choices that will help them sustain themselves and their families.

We offer a free medical clinic for people without insurance, Medicare or Medicaid. We also offer a monthly mobile Free Medical Clinic for the Saluda area. 

Jenni-Lynn Senior Living offers an engaging senior living experience in West Columbia, SC with Independent Living Cottages on Rainbow Row and Assisted Living. As a long-established community, since 1992, Jenni-Lynn offers a special home-like ambiance that makes our residents, families, visitors, and staff feel welcomed and comfortable. It is our privilege to create a safe, caring, and loving environment where our residents feel a sense of purpose, love, and "family".

Services include an adult day health care program, meal delivery, personal care and others. 

Scutta Advocacy Group provides professional advocacy and concierge life care management for seniors and those challenged by illness or disability. With wisdom, integrity, and compassion, we consider the complete lives of our clients, including medical advocacy, environmental, financial advocacy, and emotional well-being. 

Our Nurse Advocates (RN) and Physician Assistant Advocates are teamed with Certified Senior Advisors and others to gracefully provide these services.

Love House Academy's Senior Day Program is designed for retired adults over the age of 60 who are looking for a place to remain healthy in Mind, Body, and Soul.  It is for Lowcountry seniors who seek a reputable local organization to help them enjoy life to its fullest.  Our Senior Day Program is also ideal for seniors who may otherwise be isolated in their home environment.

Love House Academy's Senior Day Program is free and open to the public.  Currently we are only delivering frozen meals to senior's residence for those who qualify.  If you are in need of free meals to be delivered to your residence due to medical, health, or inabilities, please contact us for support.

Beaufort County has a large number of grandparents raising grandkids.  If you or someone you know is in need of affordable after-school and/or summer care, our Learning Academy services for students (children must be at least five years old) may be a good fit.  The Learning Academy focuses on continual educational opportunities when school is out of session and supports academics, attendance, positive behavior, and healthy choices in mind, body, and soul.  Please see our website for details or call for more information.

About Our Director:

Co-Pastor Theresa Roberts is an influential icon in her Beaufort County community through her outreach ministries and public involvement.  She attentively mentors and supports seniors, children, teens, young adults, and women.  Her passion for seeing lives changed has resulted in the $1-A-Day After School Program, the Love House Learning Academy, the Summer Learning Academy, and the Senior Day Program.  Theresa is by far a trailblazer when it comes to supporting the community and helping to bridge the gap between vulnerable people and accessible services that improve quality of life.
