Palmetto Medicaid Eligibility Consulting, LLC, (PalmettoMEC) specializes in helping South Carolina residents qualify for nursing home and in-home care Medicaid. As experienced consultants and planners, we partner with the applicant or a representative in the application process to obtain all documents needed to complete the nursing home application and meet Medicaid program requirements; we then organize the application and promptly submit it to the Medicaid agency, the SC Department of Health and Human Services (SCDHHS) for review and approval. PalmettoMEC's consultants have more than 30 years of Medicaid eligibility, policy, and case–management experience administering long-term care programs.

The Memory Health Program offers a comprehensive evaluation which includes medical, cognitive, social and physical evaluations to diagnose memory conditions and develop a plan of care for patients and their family members or care partners.

The Montgomery Speech, Language, and Hearing Clinic provides evaluation and treatment services for all ages. Focus areas include articulation, language, voice, hearing, stuttering, and everyday communication, among others. Individual and/or group programs may be recommended. Innovative services that align with research and best practices are conducted by graduate students under the direction of certified and licensed speech-language pathologists and audiologists. Individuals receiving services at the Center may be invited to participate in research during their treatment.

Lyons Prosthetics & Orthotics works to provide prosthesis and advanced orthotic braces, artificial limbs, O&P Care for amputations, orthopedic injury, and disabilities. We pride ourselves in our ability to actively listen to each person's unique needs and apply custom cutting edge prosthetic technology to meet those needs. The Greenville Area Parkinson Society (GAPS) is a local non-profit organization in the Upstate of South Carolina that offers support, education, and advocacy to people living with Parkinson's Disease, their care partners, spouses and families.

Dementia Dialogues® is a nationally registered and evidence-informed, intervention program. This 5-module training course is designed to educate community members and caregivers (formal or informal) for persons who exhibit signs and symptoms of Alzheimer’s disease and related dementias (ADRD). The modules cover valuable information and recommendations including an overview of dementia, strategies for effective communication, understanding the environment and ways to promote independence, addressing challenging behaviors, and creative problem solving. Dementia Dialogues® is offered nationwide by Certified Instructors in-person and virtually, and as a virtual self-guided training at no cost to participants. Dementia Dialogues® is provided through Office of the Study of Aging at the Arnold School of Public Health, University of South Carolina.

Aiken County Board of Disabilities provides case management to individuals of all ages in the counties listed below. We are a disabilities and special needs board that assists individuals with planning and securing the services the services they need. We serve the counties of Aiken, Barnwell, Edgefield, Greenwood, McCormick, Lexington, Orangeburg, and Saluda using a person-centered philosophy. 

We provide Guardianship, Guardian Ad Litem, Conservator, and Fiduciary services for vulnerable or incapacitated individuals.  We have been providing these services since 2015 in South Carolina (and can also provide services in North Carolina).  

Care Partners LLC is owned by Seth Zamek.  Seth holds a Masters in Healthcare Administration, is a Fellow in the American College of Healthcare Executives, is a licensed Emergency Medical Technician and a Certified Dementia Care Specialist.  Seth has over 30-years experience in the healthcare and guardianship field, most recently as the owner and Executive Director of Senior Helpers.  He has worked extensively in the Probate Courts and with Probate Court Judges and staff in South Carolina.  Seth currently serves on the Boards of Directors for the SC Alzheimer's Association, the Parkinson Association of the Carolina's, the SC Home Care and Hospice Association, and the SC Commerce Department's Catawba Regional Education Center.

Zamek is personally involved in every client case Care Partners LLC agrees to take on.  The organization serves vulnerable or incapacitated individuals of all ages and prides itself on being very responsive to the Courts, Attorneys, families petitioning the Court and clients.

Assisted Living Locators is a FREE Compassionate referral and placement service for families and seniors looking for Independent Living, Assisted Living, Memory Care or In Home Companion Care Services. Assisted Living Locators Care Advisors are in your community and work closely with you to guide you through the process from beginning to end—answering your questions and addressing your concerns, helping you feel confident and comfortable with your decisions. We are experts with the care options in your area. Once we understand your needs, we’ll create a customized plan to match and tour communities with you to help you find the best choice, and help guide you through every step of the decision-making process.

Specialize in senior placement referrals into Independent Living, Assisted Living, and Memory Care Communities for Senior Adults. We meet in person with the potential client/family to gather information about their care needs, budget, location, and personal preferences about a community. We determine what communities will meet their needs, then arrange and accompany them on tours. 

We have assessed, and continue to monitor, over 25 private-pay independent, assisted living, and memory care communities throughout Greater Columbia that offer a wide range of budgets, care levels, and amenities. We also can assist your Veteran patients and their spouses with the initial evaluation for possible Aid & Attendance benefits. Oftentimes, if they are eligible, it can make private-pay communities an affordable option. There is no charge to the patient or their family for this personalized service.
