HopeHealth Greeleyville is a community-based family practice that offers accessible and affordable primary care services in Greeleyville, SC. Services include primary care for adults as well as pediatric services. Other services range from routine visits for immunizations to pap smears and referrals from the Best Chance Network program.

We also offer free trial day passes at our day centers for those who would like to try it out, and accept many payer sources, such as Medicaid and VA.

Based on the PACE (Program of All-inclusive Care for the Elderly) model of care, our program offers frail seniors services and support needed to remain at home and in their own communities.

VR prepares and assists eligible South Carolinians with disabilities to achieve and maintain competitive employment.

The South Carolina Developmental Disabilities Council is a federally funded program that provides leadership in planning, funding, and implementing initiatives that lead to improved quality of life for people with developmental disabilities and their families through advocacy, capacity building, and systemic change.

Carrick Consulting offers Daily Money Management services to meet the needs of seniors and clients with disabilities who can handle most tasks of daily living, but find it difficult to keep up with routine paperwork and bill paying requirements. The Carrick Consulting team can help gather, analyze and organize assets; explain the client's personal financial statements and budget requirements, including a Medicare plan review, if applicable; establish a budget; manage incoming mail; set up automated bill payments; provide a monthly report on income and expenditures; handle tax preparation; and research billing disputes.

Carrick Consulting can also act as Trustee, Durable Power of Attorney or Conservator where clients cannot handle the financial tasks for themselves.  

Homewatch Caregivers is a provider of home care services in Horry and Georgetown County. We aim to preserve the dignity, protect independence, and provide peace of mind for our clients and their loved ones by providing home care services.

Palmetto Lowcountry Behavioral Health is a private hospital located on the beautiful Ashley River just outside of Charleston, South Carolina. Project VISION (Volunteers In Service In Our Neighborhoods) is the home repair initiative for the United Way of Aiken County, Inc. Home repairs are provided for low-income, disabled seniors, age 62 year or older. Senior(s) must own their home and currently occupy it.

The South Carolina Assistive Technology Program (SCATP) is a federally funded program concerned with getting technology into the hands of people with disabilities so that they might live, work, learn and be a more independent part of the community. Our goal is to enhance independence, productivity and quality of life for all South Carolinians through access to assistive technology devices and services.

Patriot Hospice & Palliative Care is a private, independent and locally-operated program. We provide Hospice, Palliative and Physician services.
