HopeHealth provides integrated health services for more than 37,000 patients in areas surrounding Aiken, Clarendon, Florence, Orangeburg, and Williamsburg counties. It was founded in 1991 as a grassroots HIV/AIDS support organization, and was designated as a Federally Qualified Health Center in 2007 to expand its services to include primary and pediatric care. As an organization, HopeHealth is now a collection of 12 locations (10 community health centers and two infectious diseases treatment and prevention centers) and nearly 60 providers covering primary care, dental care, chiropractic care, behavior health and psychiatry, massage therapy, pain management, substance abuse, and women’s health services.

The mission of the SC Respite Coalition is to expand respite opportunities in South Carolina for families of loved ones with special needs.

The Coalition’s goals are to:

• Educate the public and specific groups about the need for respite care.
• Develop strategies to provide and improve respite services for families.
• Provide support for families and providers through coordinated information resources. 

The SC Respite Coalition is the only entity in the state committed to addressing the needs for respite across the lifespan for all persons caring for a child or adult who has special needs. Lead organizations in the development of the Respite Coalition have been Family Connection of SC, Inc., the Center for Disability Resources at the USC School of Medicine, and the Institute for Families in Society at the University of South Carolina.

Palmetto Lowcountry Behavioral Health is a private hospital located on the beautiful Ashley River just outside of Charleston, South Carolina. HopeHealth offers a range of supportive services for its infectious diseases patients in coordination with their medical treatment, including medical case management. HopeHealth employs a team of medical case managers to serve as coordinators who work directly with the clinical team to manage patients’ medical needs, medication regimens, referrals, and other social services.
