The Blake at Woodcreek Farms is an Assisted Living and Memory Care community with competitive rates. The Blake at Woodcreek Farms has 66 luxurious Assisted Living suites that utilize an unmatched wellness team consisting of 24 hour nurses and personal assistants. We also have an adjoining wing with 34 suites designed to accommodate persons with memory impairments.

The Blake at Woodcreek Farms does not require a “buy-in” fee, but utilizes a month-to-month rental fee structure starting at $3925. 

Residents residing in our Assisted Living and Memory Care suites, requiring direct assistance with medications and personal care, will receive an initial evaluation by our Director of Wellness.  The development of a service plan is designed to ensure that each resident receives the assistance they require, promoting an active lifestyle.  Personal care fees for Assisted Living are based on the resident’s level of need pursuant to the initial assessment and service plan. Memory Care is all-inclusive, and the residents have no additional fees.

Griswold Home Care is a community-based service in Hilton Head that provides an affordable solution for individuals or couples who require non-medical assistance in order to remain safely in their homes. The service includes personal care, light housekeeping, meal preparation, medication reminders, companionship, along with transportation and errands if needed. Our services can be arranged for as few as 2 hours a week, or as many as 24 hours a day. Care is available seven days a week, including holidays.

We offer non-medical in-home care services that include companionship, medication reminders, bathing, dressing, shaving, grooming, oral care, skin care, feeding assistance, turning/transfers, and toileting/incontinence care. We also help with light housekeeping, laundry, meal preparation, prescription pick up, and transportation/escort to appointments or running errands.

5 Star has their own employees that are bonded and insured. Our employees go through a rigorous background check and screening.

We accept many forms of payment including: private pay, CLTC MCD, LTC insurance, VA, and Workers Comp.
