• Contracts with local council on aging agencies for services.
• Regional Aging and Disability Advisory Committee (RADAC) assist the Council of Governments in fulfilling the responsibilities of the AAA/ADRC.

The Columbia Rehabilitation Clinic, Inc. is a comprehensive outpatient rehabilitative facility dedicated to the treatment of patients in need of physical restoration. Established in 1977, our clinic was the first private outpatient physical therapy and rehabilitation facility in Columbia, South Carolina.

Our rehabilitation clinics provide physical rehabilitation and therapy services for disorders that are orthopedic, neurological, arthritic, pulmonary, cardiovascular, and sports related. All our rehabilitation services are provided on a one on one, individual basis, not group sessions, and we treat adults as well as adolescents (age 6 and up for orthopedic problems).

The primary goal of our clinics is to provide the patient, through physical therapy, an opportunity to achieve his or her maximum level of independence. Patients can be treated at the clinic on a referral from a physician, dentist, or podiatrist. A patient can also be seen for 30 days without a physician’s referral.

DayBreak Adult Care Services provides in-home, personal care service to Richland and Lexington counties. Our services include bathing, dressing, companionship, household chores, meal preparation, errands, and medication reminders. Our services are tailored to each person's needs and a free in-home consultation is provided. Sitting services are also available for patients in hospitals, nursing homes, and other types of facilities. If you don't see what you are needing, call us. Dementia Dialogues® is a nationally registered and evidence-informed, intervention program. This 5-module training course is designed to educate community members and caregivers (formal or informal) for persons who exhibit signs and symptoms of Alzheimer’s disease and related dementias (ADRD). The modules cover valuable information and recommendations including an overview of dementia, strategies for effective communication, understanding the environment and ways to promote independence, addressing challenging behaviors, and creative problem solving. Dementia Dialogues® is offered nationwide by Certified Instructors in-person and virtually, and as a virtual self-guided training at no cost to participants. Dementia Dialogues® is provided through Office of the Study of Aging at the Arnold School of Public Health, University of South Carolina.

The Department of Social Services provides protective services for children and vulnerable adults, adoption, and foster care services. It administers federal Title IV-B Child Welfare Services and Title IV-E Foster Care and Adoption Assistance programs. 

Additionally, a commodities program distributes supplemental food through a network of food banks. Other food programs provide financial assistance for child and adult care providers, homeless shelters, and summer feeding sites for children.

Protection and Advocacy for People with Disabilities Inc. (P&A) is a statewide non-profit working to protect and advance the rights of people with disabilities. P&A provides information and referrals, training, abuse and neglect investigation, legal representation, and advocacy for policy change.

Diversified Training Consultants Group provides senior care management and dementia care planning while helping families navigate long term care.

The Elder Abuse Training and Treatment Program offers free elder abuse training to organizations who work with the elderly as well as providing free telehealth therapy for South Carolinians 55 and up struggling with the effects of trauma. All services are free of charge and are available through telehealth. We are located at the Medical University of South Carolina College of Nursing.

Feonix Access to Care helps seniors and people with disabilities in Richland and Lexington counties with no-cost, door-to-door transportation to nonemergency medical appointments so they avoid the catastrophic cost of a trip to a hospital emergency department.  Feonix Access to Care combines public transit, a nonprofit transportation provider, and the medical, nonprofit, and social services community with federal grants and local matching funds to provide services that focus on individuals not served by multi-load vehicles or costly medical transport services. Medical providers and social service agencies refer patients and book rides utilizing an online booking portal.

Our Fresh Food Box is available at a cost which allows anyone to buy a box. Each box contains fresh produce and unique recipes, tips and nutrition notes. Fresh Food Boxes are filled with 9 to 11 varieties of fresh fruits and vegetables. With the contents changing weekly, it’s an affordable and exciting way to eat healthy food. Bulk buying at wholesale prices makes our boxes more affordable than buying produce at retail stores.

Our NeighborShare program is a community initiative that aims to ensure access to fresh, affordable food. The program serves individuals who need assistance ordering their Fresh Food Boxes or having boxes delivered to them by volunteers.
