As a board-certified patient advocate in South Carolina, I work closely with seniors to ensure they have a voice in their healthcare decisions and receive the care they need. I facilitate communication between seniors and their healthcare providers, assist in understanding medical conditions and treatment options, and advocate for their preferences to be respected. I also help navigate complex healthcare systems, provide emotional support during challenging situations, educate on patient rights, and collaborate with other professionals to ensure comprehensive care. By offering personalized services and empowering seniors to advocate for themselves, I strive to improve their overall well-being and quality of life and provide support for their loved ones.

 

Memory Clinic, Dementia Research

OneWell Health Care is a DPH-approved and DDSN-contracted provider offering skilled nursing, personal care, adult attendant care, in-home respite, and community services across South Carolina. Our caregivers and nurses assist with daily activities, medication management, chronic disease support, and community engagement to promote independence and well-being. We also provide veteran community care services in collaboration with the Department of Health & Human Services. With compassionate, reliable, and individualized care, we are dedicated to enhancing the quality of life for seniors, individuals with disabilities, and veterans.

James Holmes decided to open his own healthcare agency because of his passion for people and wanting to give back to the community. Our staff is dedicated to meeting the unique needs of clients and their families. An Open Hands Nursing Agency, lc in-home health care program is designed to offer flexibility, creative problem solving, and open communication to those we serve. We believe that our goal is to empower those we serve in the care of Open Hands Nursing Agency, llc in-home health care.

Advocates translate medical terminology so that you can better understand your physician's explanations and instructions so you are more likely to have positive health outcomes.

Palliative Care of South Carolina was created to provide specialized medical care for people with serious illness. Our goal is to improve the quality of life for both the patient and the family. We focus on providing relief from the symptoms and stress of advance illness by providing a team of specialists that work alongside the patient’s physicians.

With palliative care, patients will receive physical, emotional and spiritual support. The team helps to relieve pain and symptoms- such as fatigue, anxiety, shortness of breath, nausea, and depression. They also will help the patient decide the next steps; the plan and goals of care. Palliative Care of South Carolina is here to help at any stage of serious illness, regardless of age or diagnosis.

Palmetto Family Homecare is faith-based independent provider servicing the state of South Carolina.  Palmetto Family Homecare is a licensed DHEC provider, providing non-medical care in the homes of clients and within facilities.  We have a family oriented approach to our care, while specializing in Neurological Disorder Care (ALS, MS, and Parkinson).  We provide an array of services to the elderly, disabled adults, veterans, rehab recovery clients, or just anyone in the need of an extra pair of hands.   We strive to provide our families with an experience that exemplifies the 4 Keys to our family, which is Compassion, Education, Reliability and Building Relationships.

Palmetto Lowcountry Behavioral Health is a private hospital located on the beautiful Ashley River just outside of Charleston, South Carolina. We specialize in helping people effectively manage and cope with mental health and substance abuse issues. Our goal is to provide each patient with a compassionate treatment plan that focuses on creating a foundation for continued hope, healing, and recovery.

Palmetto Medicaid Eligibility Consulting, LLC, (PalmettoMEC) specializes in helping South Carolina residents qualify for nursing home and in-home care Medicaid. As experienced consultants and planners, we partner with the applicant or a representative in the application process to obtain all documents needed to complete the nursing home application and meet Medicaid program requirements; we then organize the application and promptly submit it to the Medicaid agency, the SC Department of Health and Human Services (SCDHHS) for review and approval. PalmettoMEC's consultants have more than 30 years of Medicaid eligibility, policy, and case–management experience administering long-term care programs.

Patriot Hospice & Palliative Care is a private, independent and locally-operated program. We provide Hospice, Palliative and Physician services.

We provide non-medical in home care services.

We provide loving care in the privacy of your home.

 Our residents have access to a choice of home health agencies that provide regular personal care assessments. Staff members who are on call 24 hours a day work with residents to tailor plans for all levels of care needed, with services including medication reminders and administration, incontinence care, special nutritional counseling and escort and transfer assistance available if and when needed. Residents also can obtain bathing, dressing and grooming assistance and medication administration as necessary.

Physical, occupational and speech therapies are available on site as conveniences to our residents, who also have access to psychotherapy services. Wellness, walking, balance and exercise classes also are offered on a regular basis.

Non medical home care agency

Plan Home Care is a non-skilled in-home care provider of upstate SC.

PLAN Home Health Care is the premier champion of the in-home care experience. We strive for excellence in caring for our clients, and offer an extensive list of services to give our clients superior care and total comfort. Our ultimate goal at PLAN is for our clients and their loved ones to feel like they are an extension of our own family.

We provide gentle breathing exercises and stretching exercises that can be done by sitting on a chair or on a recliner. It is a good way to relieve stress, and stay healthy. 

Premier Counseling, LLC provides geriatric psychiatry to include psychotherapy (weekly therapy sessions) and medication management of psychotropic medication to address symptoms related to depression, anxiety, bipolar disorder, schizoaffective disorder, dementia w/behavior disturbances, grief and loss, adjustment, and phase of life concerns. 

Based on the PACE (Program of All-inclusive Care for the Elderly) model of care, our program offers frail seniors services and support needed to remain at home and in their own communities.

Professional Case Management was founded with the goal of providing the best possible care in the best possible place – the home. PCM caregivers follow the core principles of FOCUS, EXPERIENCE, QUALITY and COMPASSION to provide in-home care services with the highest standards and ethics in the industry.
