Based on the PACE (Program of All-inclusive Care for the Elderly) model of care, our program offers frail seniors services and support needed to remain at home and in their own communities.

Prisma Health SeniorCare - PACE provides All-inclusive Care for the Elderly living in Richland and Lexington counties.  A program for people over the age of 55 living with a disability.  The goal of the program is to keep people in the community for as long as safely possible.  The program is centered on the belief that the best outcomes happen when seniors are served in their own communities.  To meet this goal, Prisma Health SeniorCare - PACE focuses on both chronic care needs and preventive care with a medical and holistic approach. 

Program of All-Inclusive Care for the Elderly designed to provide a variety of services to the frail elderly. By providing all medical care and supportive services, the program is able to offer care and services to seniors with chronic care needs, while encouraging independence at home for as long as possible.

For almost 40 years, Wellhaven Home Care has been working to ensure that you are able to abide at home during a time of need. Experienced caregivers enable our clients to thrive in the comfort of their own home by preparing meals, tidying living spaces, running errands, and assisting with basic life functions, like bathing and dressing.

Sometimes your concerns are more complex – a child with disabilities, a spouse who requires significant care as he or she recovers from a procedure, or navigating life with a difficult diagnosis such as Alzheimer's.
