- Medical conditions and functional limitations
- ADL/IADL support needs
- Personal care needs
- Safety and environmental considerations
- Client preferences and goals
- A general intake assessment is completed for all clients, including:
- Service Determination
- Based on the intake assessment and authorized care plan, clients may be enrolled in one or more services offered by the agency, including but not limited to:
- Personal Care
- Homemaker Services
- Companion Services
- Respite Care
- Other approved non-medical support services
- Services are not limited to a single category when needs overlap.
