    Intake Procedure

    Purpose:

    To ensure all clients receiving services through Harrison Family Care Services are appropriately assessed and enrolled in services based on individual needs, preferences, and authorized care plans.


     

    Procedure:


     

    1. Referral & Initial Contact
       
      • Referrals are received through GetCareSC (SCDOA), community partners, or self-referral.
      • Initial contact is made with the client and/or authorized representative to explain available services and begin the intake process.

    2.  
    3. Comprehensive Intake Assessment
       
      • A general intake assessment is completed for all clients, including:
         
        • Demographics and contact information
        • Medical conditions and functional limitations
        • ADL/IADL support needs
        • Personal care needs
        • Safety and environmental considerations
        • Client preferences and goals

    4.  
    5. Service Determination
       
      • Based on the intake assessment and authorized care plan, clients may be enrolled in one or more services offered by the agency, including but not limited to:
         
        • Personal Care
        • Homemaker Services
        • Companion Services
        • Respite Care
        • Other approved non-medical support services

      •  
      • Services are not limited to a single category when needs overlap.

    6.  
    7. Care Plan Development
       
      • An individualized care plan is developed outlining all approved services, frequency, and tasks.
      • The care plan aligns with SCDOA authorization and client needs.

    9. Service Initiation
       
      • Caregivers are assigned based on client needs, availability, and compatibility.
      • Services begin upon authorization, caregiver assignment, and completion of required documentation.

    11. Ongoing Review & Updates
       
      • Client needs are reviewed regularly.
      • Services may be adjusted, added, or discontinued based on reassessment, changes in condition, or updated authorization.
