    Gracefully Diverse In Home Care provides essential, non-medical support to help seniors and individuals with disabilities manage their daily physical needs. Our Personal Care services focus on both direct, hands-on assistance and verbal cueing to ensure safety and promote self-sufficiency within the home.

    Core Assistance Areas:

    • Activities of Daily Living (ADLs): Direct support with essential self-care, including bathing (bed baths, tub, or shower), dressing, grooming, and oral hygiene.
    • Mobility & Transfers: Safe assistance with ambulation (walking), getting in and out of bed, and positioning to prevent falls.
    • Toileting & Incontinence Care: Respectful help with toileting needs and maintaining continence.
    • Nutritional Support: Hands-on assistance with eating and feeding for clients with physical or cognitive limitations.
    • Health Monitoring: Routine monitoring of "vital-like" signs that a family member would typically observe, such as checking pulse, respiration, and temperature.

    Our caregivers are trained to provide this care with the utmost respect for the client’s dignity, ensuring they feel comfortable and supported in their most private routines.

    Open Mondays thur Fridays 9:00 AM to 6:00 PM, except holidays. And Saturdays from 11am to 3pm.

    Intake Process

    Getting started with Gracefully Diverse In Home Care is designed to be seamless and supportive for the family. Our process includes:

    1. Initial Consultation: A brief phone or in-person discussion to understand your needs and determine if our services are the right fit.
    2. In-Home Assessment: A comprehensive visit by our care team to evaluate the home environment for safety and discuss specific daily requirements, health goals, and cultural preferences.
    3. Custom Care Plan: We develop a personalized plan of care that addresses physical needs, social engagement, and any specialized requirements (such as Veteran-specific care).
    4. Caregiver Matching: We introduce a caregiver who best fits the client’s personality and care needs to ensure a comfortable and lasting connection.

    Insurance & Payment Options

    • Medicaid & CLTC: Contracted provider for South Carolina Community Long Term Care (CLTC) and Medicaid.
    • Workers’ Compensation & Specialty Networks: We are credentialed with Homelink and Paradigm, allowing us to provide care for injured workers covered by most major national insurance carriers and third-party administrators.
    • Private Pay: Flexible out-of-pocket billing options.
    • Veterans Affairs (VA) Care:
    • VA Community Care: We are currently on the waitlist for the VA Community Care Network (VACCN) due to market saturation.
      • Veterans Care Agreements (VCA): To ensure our local Veterans do not experience a gap in services, we are ready and willing to initiate and follow through on a Veterans Care Agreement (VCA). This allows us to work directly with the local VA Medical Center to provide authorized care for Veterans in need when network options are insufficient.
    • Long-Term Care Insurance: We accept most major policies (please contact us for verification).
