    Based on the results of a comprehensive care management assessment, Margaret’s full-service program spans an array of services tailored to the individual client. Care management services can involve: offering referrals, and then helping to screen, assemble, and monitor a team of medical, home-based and legal professionals for both caregiver and loved one, and coordinating their services; assisting with health insurance, bill paying and other money management; accompanying clients and acting as an advocate on visits to physicians, attorneys, financial advisers and others; arranging for contacts with community agencies and services, such as support groups, educational programs, monetary grants and matching funds programs, respite care, and counseling; providing immediate crisis intervention; establishing and maintaining liaisons with in- and out-of-town family members; acting as an appointed Health Care Agent, or as an adviser to that agent; helping with moving from home to a retirement complex or nursing facility; acting as advocate during a planned or unplanned hospitalization or other short-term stay; and discussing end-of-life decisions and implementation.

    Intake Procedure

    Margaret will lay out a plan for the coming months that includes a contract
    and anticipated expenses. The plan begins with a 90-minute professional assessment of an individual’s current physical, medical, emotional and social strengths and challenges, as well as support systems in place. Following the assessment, Margaret will spend 90 minutes creating a plan of action aimed at maintaining independence and autonomy, assuring safety and security, and enlisting the finest health and personal services.

    Margaret offers two approaches to aging life care: A comprehensive management program that covers all aspects of current daily life as well as future planning, and a retainer plan that emphasizes planning for future needs, taking current circumstances into consideration.

Description of Organization

Margaret Kunes, MHA, CMC is an Aging Life Care Manager who owns and operates a small practice offering highly personalized decision support for individuals and families face with managing chronic illness, particularly cognitive impairment, or more general matters related to aging and planning for the future.

ALCC also offers assistance with exploring retirement community options, medication management, home care management, coordinating and attending medical and other business appointments, crisis intervention, client and caregiver education and support, and additional related services based on needs of the individual situation. Based on the understanding that she and her clients establish, Margaret draws on her extensive Charleston network to recommend carefully selected local physicians, attorneys, financial advisers, home health and respite support, residential services and more — as needed. And, as family
members arrive at crossroads in their caregiving, she helps them
come to the decisions, big and small, that “feel right.”

Nationally certified as a professional care manager, and owner of
Aging Life Care Charleston, LLC, Margaret has built a reputation
for providing sensitive, knowledgeable guidance to individuals
and caregiving family members dealing with illnesses, such as Alzheimer’s and similar dementias. Margaret works with clients to explore opportunities to plan ahead of a need for a graceful and proactive approach to future care needs in addition to providing support in challenging situations that can create medical, financial, legal — and purely practical — dilemmas that can seem overwhelming.
