    Assessment

    Care management services can involve: offering referrals, and then helping to screen, assemble, and monitor a team of medical, home-based, and legal professionals for both caregivers and loved ones, coordinating their services; assisting with health insurance, bill paying, and other money managers.

    Accompanying clients and acting as an advocate on visits to physicians, attorneys, financial advisers; arranging for contacts with community agencies and services, such as support groups, educational programs, monetary grants and matching funds programs, respite care, and counseling; providing immediate crisis intervention.

    Helping with moving from home to a retirement complex or nursing facility; acting as an advocate during a planned or unplanned hospitalization or another short-term stay; and discussing end-of-life decisions and implementation.

    Intake Procedure

    Margaret will lay out a plan for the coming months that includes a contract
    and anticipated expenses. The plan begins with a 90-minute professional assessment of an individual’s current physical, medical, emotional and social strengths and challenges, as well as support systems in place. Following the assessment, Margaret will spend 90 minutes creating a plan of action aimed at maintaining independence and autonomy, assuring safety and security, and enlisting the finest health and personal services.

    Margaret offers two approaches to aging life care: A comprehensive management program that covers all aspects of current daily life as well as future planning, and a retainer plan that emphasizes planning for future needs, taking current circumstances into consideration.

Description of Organization

Margaret Kunes, MHA, CMC is an Aging Life Care Manager who operates a small practice offering highly personalized decision support for individuals and families faced with managing chronic illness, particularly cognitive impairment, or more general matters related to aging and planning for the future.

ALCC also offers assistance with exploring retirement community options, medication management, home care management, coordinating and attending medical and other business appointments.

Open Mondays through Friday, 9:00 AM - 4:00 PM. In some cases, evening and weekend appointments may be available. Special arrangements can be made if 24 hour support is required.
