Care Management
We have a team of experienced Case Managers and Social Workers with extensive experience in assisting families, veterans, disabled adults and seniors navigate life planning and their overall care. Our Case Managers and Social Workers are advocates for your loved one, an extra set of ears and eyes when you are not able to do so; while assisting in making informed decisions that add to their quality of life.
As Case Managers we advocate, research, coordinate and manage care. Whether at home, in the hospital, in an assisted living facility or wherever you consider home to be, our Case Managers will advocate, act on your behalf and in your best interest.

Palmetto Family Homecare is faith-based independent provider servicing the state of South Carolina. Palmetto Family Homecare is a licensed DHEC provider, providing non-medical care in the homes of clients and within facilities. We have a family oriented approach to our care, while specializing in Neurological Disorder Care (ALS, MS, and Parkinson). We provide an array of services to the elderly, disabled adults, veterans, rehab recovery clients, or just anyone in the need of an extra pair of hands. We strive to provide our families with an experience that exemplifies the 4 Keys to our family, which is Compassion, Education, Reliability and Building Relationships.
