We are a non-medical home care company that provides assistance to adults that are elderly, the handicap and disabled, and diagnosed with cognitive disabilities. We have CNAs and home care aides that will assist with their ADLs (Activities of Daily Living). We accept Medicaid clients.
Hours of Operation
Office Hours- Mon-Fri 9am to 5pm
Service Hours: 4hr (minimum) up to 24hrs a day/ 7 days a week

Intake Procedure
After initial referral received, our supervising RN with make a first visit to the client/family. Our company services will be further discussed and home/client assessment completed.
Hours of service/type of service needed ( based off assessment) is discussed and agreed upon. Contract for service and service agreement is reviewed, explained, and signed by client or their legal representative.
* If a client has medicaid, their medicaid number is checked and verified through Healthy Connections web portal. The approval for home care and service hour amount is also verified through Healthy Connections. Once verified, we follow scdhhs protocol for accepting new clients for the start of approved services.
All collected client data is logged under the client profile section in Axis Care ( our home care business platform).

We are a non-medical home care company that provides assistance to adults that are elderly, the handicap and disabled, and diagnosed with cognitive disabilities. We have CNAs and home care aides that will assist with their ADLs (Activities of Daily Living). Our service allows people to remain in the comfort or their homes for as long as possible in hopes of preventing unnecessary institutionalization. Patricia Home Care also offers sitter/companionship services for families that have a loved one in the hospital, nursing home, assisted living , or at home. Our company aims to provide the safest and best care possible while maintaining the dignity and respect of our clients. Our company also aims to help provide a "piece of mind" and reassurance that their loved one is taken care of and safe.
