PurFoods, LLC dba Mom’s Meals (“Mom’s Meals”) is a USDA-inspected food preparation and home-delivered meal service dedicated to providing delicious meals to our customers nationwide. Mom’s Meals strives to be the leading provider of aging, disabled and patient nutrition solutions that preserve health, help combat chronic disease, support recuperation and nourish independence for high-quality living at home. Mom's Meal's mission is to help keep seniors & individuals with disabilities in the comfort of their homes as long as possible by delivering nutritious meals directly to their homes. We have grown to provide daily in-home nutrition support for over 180,000 unique clients across the country. We utilize a HIPAA-compliant database that tracks each client's ordering history, feedback, notes from Case Managers, etc. Every client's order is maintained in their respective account and includes the number of meals for each delivery and delivery verification. 

The Montgomery Speech, Language, and Hearing Clinic provides evaluation and treatment services for all ages. Focus areas include articulation, language, voice, hearing, stuttering, and everyday communication, among others. Individual and/or group programs may be recommended. Innovative services that align with research and best practices are conducted by graduate students under the direction of certified and licensed speech-language pathologists and audiologists. Individuals receiving services at the Center may be invited to participate in research during their treatment.

As a board-certified patient advocate in South Carolina, I work closely with seniors to ensure they have a voice in their healthcare decisions and receive the care they need. I facilitate communication between seniors and their healthcare providers, assist in understanding medical conditions and treatment options, and advocate for their preferences to be respected. I also help navigate complex healthcare systems, provide emotional support during challenging situations, educate on patient rights, and collaborate with other professionals to ensure comprehensive care. By offering personalized services and empowering seniors to advocate for themselves, I strive to improve their overall well-being and quality of life and provide support for their loved ones.

OneWell Health Care is a DPH-approved and DDSN-contracted provider offering skilled nursing, personal care, adult attendant care, in-home respite, and community services across South Carolina. Our caregivers and nurses assist with daily activities, medication management, chronic disease support, and community engagement to promote independence and well-being. We also provide veteran community care services in collaboration with the Department of Health & Human Services. Palliative Care of South Carolina was created to provide specialized medical care for people with serious illness. Our goal is to improve the quality of life for both the patient and the family. We focus on providing relief from the symptoms and stress of advance illness by providing a team of specialists that work alongside the patient’s physicians.

With palliative care, patients will receive physical, emotional and spiritual support. The team helps to relieve pain and symptoms- such as fatigue, anxiety, shortness of breath, nausea, and depression. They also will help the patient decide the next steps; the plan and goals of care. Palliative Care of South Carolina is here to help at any stage of serious illness, regardless of age or diagnosis.

Palmetto Family Homecare is faith-based independent provider servicing the state of South Carolina.  Palmetto Family Homecare is a licensed DHEC provider, providing non-medical care in the homes of clients and within facilities.  We have a family oriented approach to our care, while specializing in Neurological Disorder Care (ALS, MS, and Parkinson).  We provide an array of services to the elderly, disabled adults, veterans, rehab recovery clients, or just anyone in the need of an extra pair of hands.   We strive to provide our families with an experience that exemplifies the 4 Keys to our family, which is Compassion, Education, Reliability and Building Relationships.

Palmetto Medicaid Eligibility Consulting, LLC, (PalmettoMEC) specializes in helping South Carolina residents qualify for nursing home and in-home care Medicaid. As experienced consultants and planners, we partner with the applicant or a representative in the application process to obtain all documents needed to complete the nursing home application and meet Medicaid program requirements; we then organize the application and promptly submit it to the Medicaid agency, the SC Department of Health and Human Services (SCDHHS) for review and approval. Non medical home care agency

We provide gentle breathing exercises and stretching exercises that can be done by sitting on a chair or on a recliner. It is a good way to relieve stress, and stay healthy. 

Premier Counseling, LLC provides geriatric psychiatry to include psychotherapy (weekly therapy sessions) and medication management of psychotropic medication to address symptoms related to depression, anxiety, bipolar disorder, schizoaffective disorder, dementia w/behavior disturbances, grief and loss, adjustment, and phase of life concerns. 

Prisma Health SeniorCare - PACE provides All-inclusive Care for the Elderly living in Richland and Lexington counties.  A program for people over the age of 55 living with a disability.  The goal of the program is to keep people in the community for as long as safely possible.  The program is centered on the belief that the best outcomes happen when seniors are served in their own communities.  To meet this goal, Prisma Health SeniorCare - PACE focuses on both chronic care needs and preventive care with a medical and holistic approach. 

Professional Case Management was founded with the goal of providing the best possible care in the best possible place – the home. PCM caregivers follow the core principles of FOCUS, EXPERIENCE, QUALITY and COMPASSION to provide in-home care services with the highest standards and ethics in the industry. PCM’s dedicated and knowledgeable professionals deliver a full range of services from monthly visits or help with daily activities to extensive, round-the-clock continuous care. In the comfort of home, clients and families are able to maintain the independence and quality of life they have earned.

The Lexington County Sheriff's Department has forged a strong relationship with Project Lifesaver. This partnership allows us to use Project Lifesaver's technology to quickly track and locate individuals who are prone to wander from their homes.  You may contact Sgt. When you enroll your loved one in the program, you will receive a small personal Project Lifesaver transmitter.   This transmitter is worn on the wrist or ankle and emits an individualized tracking signal. If an enrolled Project Lifesaver client goes missing, the caregiver notifies the Department and deputies respond to the wanderer's area. Most clients who wander are found a few miles from home, and search times are reduced from hours and days to minutes. Recovery times for PLI clients average 30 minutes which is a 95 percent less time than standard operations.

Hospice care offers peace and comfort to patients with life-limiting illness through a family-focused approach. Palliative care is specialized for those with an acute or chronic illness and is focused on symptom management and stress relief.
