We provide non-medical home care for seniors, people with disabilities, those recovering from illness, injury, or surgery, mothers just home from the hospital, veterans, and anyone who needs a little extra help. We offer companion care and personal care services.

Assisted Living Locators is a FREE referral/placement service for seniors looking for Independent Living, Assisted Living, Memory Care or In Home Companion Care Services.

Carolina Center for Behavioral Health provides assistance in psychiatric and substance abuse treatment. Our programs focus on  treatments for inpatient hospitalization and outpatient.

The mission of Bamberg County Office on Aging is to contribute to the well-being and improve the quality of life for older adults in Bamberg County and to provide activities and services that promote independence and continuing participation in the community.

HopeHealth Lake City is a primary care and family medicine doctor’s office in Lake City, SC. HopeHealth is a community-based health center providing quality primary health care that is accessible and affordable. Services include primary care for men and women as well as pediatric and family medicine services. Other services range from routine visits for immunizations and physicals to pap smears, including referrals from the Best Chance Network and WISEWOMAN programs.

HopeHealth also provides youth immunizations, physicals, and wellness exams in Lake City through its school-based program.

We are a Non-Profit 501 3c Organization. We service Home Delivered meals, Group Dining Meals, Health and Human Services and Transportation Services.

HopeHealth provides integrated health services for more than 37,000 patients in areas surrounding Aiken, Clarendon, Florence, Orangeburg, and Williamsburg counties. It was founded in 1991 as a grassroots HIV/AIDS support organization, and was designated as a Federally Qualified Health Center in 2007 to expand its services to include primary and pediatric care. As an organization, HopeHealth is now a collection of 12 locations (10 community health centers and two infectious diseases treatment and prevention centers) and nearly 60 providers covering primary care, dental care, chiropractic care, behavior health and psychiatry, massage therapy, pain management, substance abuse, and women’s health services.

Lexington County Recreation & Aging Commission is a special purpose district whose mission is to improve the quality of life for all citizens through creative, meaningful, life enriching experiences. This is done through the provision of program services, facilities, and education.

As a geriatric care consultant and gerontologist, Occasional Angel help families navigate through new and unknown issues common in the aging process.

Based on the PACE (Program of All-inclusive Care for the Elderly) model of care, our program offers frail seniors services and support needed to remain at home and in their own communities.

Priority Home Care LLC. (PHC) offers services to individuals who want to remain at home but are in need of help with their personal and household care.

Hospice & Palliative Care of the Piedmont is your community non-profit network of care that provides expert life-changing health care for those living with advanced illnesses. Has been a non-profit, community based hospice since 1981. Employs more than 68 trained staff including 4 Nurse Practitioners, 15 licensed nurses, 4 certified Hospice and Palliative Care nurses, 3 licensed social workers, 3 physicians, and 1 certified Hospice & Palliative Care Administrator.

• Bereavement Services 

We are a comprehensive resource for those in Aiken, Allendale, Bamberg, Barnwell, Calhoun and Orangeburg counties. The Lower Savannah Council of Governments  helps develop, write and administer grants; provides research and data for planning and decision-making; offers access to the Aging, Disability & Transportation Resource Center and coordinates the region’s workforce development programs.

Serving seniors 60+

 Our residents have access to a choice of home health agencies that provide regular personal care assessments. Staff members who are on call 24 hours a day work with residents to tailor plans for all levels of care needed, with services including medication reminders and administration, incontinence care, special nutritional counseling and escort and transfer assistance available if and when needed. Residents also can obtain bathing, dressing and grooming assistance and medication administration as necessary.

Physical, occupational and speech therapies are available on site as conveniences to our residents, who also have access to psychotherapy services. Wellness, walking, balance and exercise classes also are offered on a regular basis.
