We also provide youth transition services. As an organization staffed and board-directed by a majority of people with disabilities ourselves, we understand the needs of youth, their families, and the professionals supporting them in ways that other agencies may not.

FCCOA is a nonprofit based in Winnsboro, SC, that collaborates with organizations in Fairfield County on providing senior services. FCCOA receives funding and guidance from federal, state, local governments and generous donors to carry out its mission of providing senior services and resources to improve the quality of life for the county's senior population.

Vision - Fairfield County Council on Aging aims to address the physical, social, mental and emotional needs of the elder population by equipping them with knowledge, assisting them with nutritional requirements and providing access to programs and services designed to elevate their well-being.  Simplicity Pharmacy Services and Wellness Center, inc. is a Concierge Pharmacy Consulting Practice comprised of licensed and Board-Certified Geriatric Pharmacists who work with Seniors, Caregivers, and their Healthcare Providers to design 

customized medication management and wellness programs so that Seniors can age in place. We will do a Medication Risk Assessment to check for drug interactions and appropriate dosing, perform Falls Risk Assessments, and look for opportunities to safely remove or decrease any unnecessary or potentially harmful medications. Our overall mission is to Simplify Medication Regimens, Promote Wellness, and Empower Families.

LCFS provides Home Care service to the Low Country of South Carolina, offering a solution for individuals who want to maintain their independence at home but need assistance due to short- or long-term disability, illness or simply the natural aging process.

Visiting Angels provides non medical home care personnel and companion services that allow people to maintain the independence of their daily routines and familiar surrounding in the privacy of their own homes

Home Sweet Home In-Homecare is a licensed DHEC provider, providing non-medical care in the homes of clients and within facilities. Home Sweet Home In-Home Care Services provides loving caregivers to help seniors and adults remain independent and living at home. We also provide assistance in choosing the right independent/assisted living/memory care community for you or your elderly loved one.

Lexington County Recreation & Aging Commission's Aging Program is a special purpose district whose mission is to improve the quality of life of older adults throughout Lexington County through creative, meaningful, life enriching experiences. This is done through the provision of program services, facilities, and education. Some programs and services may depend on available funds, an individual's severity of need, and the geographic location of the individual. Low and no-cost services are available. 

Our Helping Hands is a patient advocacy organization that services seniors of Charleston South Carolina. We are dedicated to assisting you in becoming an active participant in your own health care and empower you to ask the right questions of your providers. Advocates translate medical terminology so that you can better understand your physician's explanations and instructions so you are more likely to have positive health outcomes.

Short term acute care behavioral health hospital with an average length of stay ranging from 7-10 days. We offer advanced programming and services for the treatment of behavioral health illness for adult patients. Our hospital offers advocacy and assistance for acute care services related to anxiety, depression, schizophrenia, bipolar disorder, and psychotic diagnoses. Our priority is aimed at the best patient care possible through an integrated clinical practice. Midlands Behavioral Health Hospital professionals are trained with high ethical standards and are committed to providing high quality care while maintaining respect and exercising integrity for our patients.  

At Dorchester Seniors, Inc., adults 50+ find fitness classes, social activities, nutrition & dining, transportation, and our Second Change Program - all designed to help you live well every day in Dorchester County. Our home delivered meal program provides fresh, dietitian-approved lunches delivered directly to the doorsteps of homebound seniors. Low-cost and no-cost services are available.

The South Carolina Department of Disabilities and Special Needs (DDSN) is the state agency that plans, develops, coordinates and funds services for residents with various severe, lifelong disabilities. Populations served include individuals with traumatic brain injuries and spinal cord injuries, autism spectrum disorder, and intellectual disabilities.

Shoreline Behavioral Health Services addresses the problem of alcohol and substance abuse in Horry County by providing individualized, recovery based treatment and community prevention services.

We are specialized in aiding individuals with dementia, Alzheimer’s, Parkinson’s and other conditions involving chronic loss of memory and motor skills. With this in mind, our staff has been properly trained and our facilities are fully secured, while remaining inviting and empowering for our members. Whether it’s Adult Day Center or Home Care services, we have you covered providing your loved ones with top care. It should be noted that our Adult Day Center and Home Care services can be combined to best fit you and your loved one’s individual needs and budget. Elite Day Center members also receive a discount for our Home Care services.

Catawba Area Agency on Aging (AAA) serves as the regional aging focal point and resource connection for seniors, adults with disabilities, caregivers, families and professionals
