Specialize in senior placement referrals into Independent Living, Assisted Living, and Memory Care Communities for Senior Adults. We meet in person with the potential client/family to gather information about their care needs, budget, location, and personal preferences about a community. We determine what communities will meet their needs, then arrange and accompany them on tours. 

We have assessed, and continue to monitor, over 25 private-pay independent, assisted living, and memory care communities throughout Greater Columbia that offer a wide range of budgets, care levels, and amenities. We also can assist your Veteran patients and their spouses with the initial evaluation for possible Aid & Attendance benefits. Oftentimes, if they are eligible, it can make private-pay communities an affordable option. There is no charge to the patient or their family for this personalized service.

We provide Guardianship, Guardian Ad Litem, Conservator, and Fiduciary services for vulnerable or incapacitated individuals.  We have been providing these services since 2015 in South Carolina (and can also provide services in North Carolina).  

Care Partners LLC is owned by Seth Zamek.  Seth holds a Masters in Healthcare Administration, is a Fellow in the American College of Healthcare Executives, is a licensed Emergency Medical Technician and a Certified Dementia Care Specialist.  Seth has over 30-years experience in the healthcare and guardianship field, most recently as the owner and Executive Director of Senior Helpers.  He has worked extensively in the Probate Courts and with Probate Court Judges and staff in South Carolina.  Seth currently serves on the Boards of Directors for the SC Alzheimer's Association, the Parkinson Association of the Carolina's, the SC Home Care and Hospice Association, and the SC Commerce Department's Catawba Regional Education Center.

Zamek is personally involved in every client case Care Partners LLC agrees to take on.  The organization serves vulnerable or incapacitated individuals of all ages and prides itself on being very responsive to the Courts, Attorneys, families petitioning the Court and clients.

Services can include personal care (bathing, dressing, grooming). Our services are commonly used by seniors, individuals with disabilities, or those recovering from illness or surgery.

Free program for drivers where they remain in their vehicles & trained technicians go through 12 safety checks to make sure they fit in their vehicles & are safe to drive.

We check all lights, mirror adjustments, seat belts, head restraints, seat location as far as seeing over the dash & ability to reach petals. A trained Occupational Therapist will be there most of the time to help anyone that has problems getting in or out of their vehicles, etc.

We provide companionship to our aging population and support workforce caregivers who care for our aging population. The organization aims to help older adults to live socially engaged lives that reduces negative outcomes such as loneliness, isolation and depression. These goals are achieved by providing social engagement, volunteer opportunities and intergenerational programming for all seniors. 

Caring Oasis HomeCare is a local HomeCare provider with a dedicated staff and owner who enjoys being personally involved in caring for their clients. They are assist with many activities of daily living and will ensure that your loved one's needs are looked after. 

Caring Healthcare provides in home personal care services to elderly, disabled, and special needs children. We offer assistance with personal care, meal preparation, light housekeeping, companionship, supervision, transportation of adults, errands, assistance with ambulation & transfers.  In addition to in home care we also provide private sitting for clients during hospitalizations, while staying in a rehab facility after hospitalization, and for added supervision and assistance while staying in an assisted living facility.  We are available to provide services from 2 to 24 hours daily as needed.

Carolina Center for Behavioral Health provides assistance in psychiatric and substance abuse treatment. Our programs focus on  treatments for inpatient hospitalization and outpatient.
