Certified in 2012, Pathway Hospice was born out of the vision of a family owned company who realized the value of hospice care and wanted it for their families and neighbors
Pathway Hospice is community based, offering comprehensive hospice and palliative care, caregiver education, individual and group counseling, spiritual support and many other programs to those it serves regardless of race, age, faith, diagnosis or financial circumstances.

Patriot Hospice & Palliative Care is a private, independent and locally-operated program. We provide Hospice, Palliative and Physician services.

We provide non-medical in home care services.

We provide loving care in the privacy of your home.

 Our residents have access to a choice of home health agencies that provide regular personal care assessments. Staff members who are on call 24 hours a day work with residents to tailor plans for all levels of care needed, with services including medication reminders and administration, incontinence care, special nutritional counseling and escort and transfer assistance available if and when needed. Residents also can obtain bathing, dressing and grooming assistance and medication administration as necessary.

Physical, occupational and speech therapies are available on site as conveniences to our residents, who also have access to psychotherapy services. Wellness, walking, balance and exercise classes also are offered on a regular basis.

Plan Home Care is a non-skilled in-home care provider of upstate SC.

We provide gentle breathing exercises and stretching exercises that can be done by sitting on a chair or on a recliner. It is a good way to relieve stress, and stay healthy. 

Premier Counseling, LLC provides geriatric psychiatry to include psychotherapy (weekly therapy sessions) and medication management of psychotropic medication to address symptoms related to depression, anxiety, bipolar disorder, schizoaffective disorder, dementia w/behavior disturbances, grief and loss, adjustment, and phase of life concerns. 

Based on the PACE (Program of All-inclusive Care for the Elderly) model of care, our program offers frail seniors services and support needed to remain at home and in their own communities.

ResCare HomeCare- Greenville, is dedicated to impacting communities for the better with a mission to help people live their best life.  ResCare offer services to people with disabilities, home care services, specialized high-acuity neuro-rehab in community settings, a career center workforce contractor in the U.S.

Richard M. Campbell Veterans Nursing Home is managed by the South Carolina Department of Mental Health and operated by HMR Governmental Services on behalf of the State.Admission to the Richard M. Campbell Veterans Nursing Home is limited to Veterans who have been separated from the U.S. Armed Forces under honorable conditions and qualify as South Carolina residents who are in need of skilled or intermediate nursing home care. Eligible Veterans must have served on active duty. 

Senior Action is a nonprofit organization serving adults aged 55 and older in Greenville County. We offer a wide range of programs and services, including meals, exercise classes, social activities, and more at our six locations. We also provide home-based services such as transportation to our centers, medical transportation, and yard care assistance. Volunteer opportunities available. 

We are the only full placement service that can assist with Medicaid placements for assisted living and skilled care!

 Simplicity Pharmacy Services and Wellness Center, inc. is a Concierge Pharmacy Consulting Practice comprised of licensed and Board-Certified Geriatric Pharmacists who work with Seniors, Caregivers, and their Healthcare Providers to design 

customized medication management and wellness programs so that Seniors can age in place. We will do a Medication Risk Assessment to check for drug interactions and appropriate dosing, perform Falls Risk Assessments, and look for opportunities to safely remove or decrease any unnecessary or potentially harmful medications. Our overall mission is to Simplify Medication Regimens, Promote Wellness, and Empower Families.

SCLS is a statewide law firm providing free legal services to low income citizens of South Carolina.

We provide free telephone equipment for individuals with hearing and/or speech impairments.

The South Carolina Developmental Disabilities Council is a federally funded program that provides leadership in planning, funding, and implementing initiatives that lead to improved quality of life for people with developmental disabilities and their families through advocacy, capacity building, and systemic change.

We serve individuals with spinal cord injuries to assist them in finding resources for their needs, questions, and concerns.

The mission of the SC Respite Coalition is to expand respite opportunities in South Carolina for families of loved ones with special needs.

The Coalition’s goals are to:

• Educate the public and specific groups about the need for respite care.
• Develop strategies to provide and improve respite services for families.
• Provide support for families and providers through coordinated information resources. 

The SC Respite Coalition is the only entity in the state committed to addressing the needs for respite across the lifespan for all persons caring for a child or adult who has special needs. Lead organizations in the development of the Respite Coalition have been Family Connection of SC, Inc., the Center for Disability Resources at the USC School of Medicine, and the Institute for Families in Society at the University of South Carolina.

Straight Road International is a 501(c)(3) non-profit organization that serves communities through education and programs in financial and health literacy. Through our StraightAge Institute, we serve the diverse needs of older adults and their caregivers. We are engaged in memory screenings (non-diagnostic) and other community programs. We deliver educational content, training, and resources on various topics to older adults and their networks.
