Social services - we employ certified social workers to ensure that our patients are getting the benefits they're entitled to.
Health care services - we employ licensed registered nurses for the safety and well-being of our patients.
Transportation services - we take patients to medical appointments and on occasional outings.
Therapeutic recreation services - we offer on-site physical, occupational and speech therapy, and we work with preferred partners.

SLHCA provides social services to seniors in the Columbia, South Carolina area. They employ certified social workers to ensure that our patients are getting the benefits they're entitled to. Pathway Health and Wellness is a dedicated home health service company providing non-medical care to clients in Columbia South Carolina. 

Certified in 2012, Pathway Hospice was born out of the vision of a family owned company who realized the value of hospice care and wanted it for their families and neighbors.

We are a non-medical home care company that provides assistance to adults that are elderly, the handicap and disabled, and diagnosed with cognitive disabilities.  We have CNAs and home care aides that will assist with their ADLs (Activities of Daily Living).  Our service allows people to remain in the comfort or their homes for as long as possible in hopes of preventing unnecessary institutionalization.   Patricia Home Care also offers sitter/companionship services for families that have a loved one in the hospital, nursing home, assisted living , or at home.  Our company aims to provide the safest and best care possible while maintaining the dignity and respect of our clients.  Our company also aims to help provide a "piece of mind" and reassurance that their loved one is taken care of and safe.

Patriot Hospice & Palliative Care is a private, independent and locally-operated program. We provide Hospice, Palliative and Physician services.

Pee Dee Community Project, LLC. is a non-profit organization that is instrumental in providing a variety of programs that serve community members of every age and background. Our mission is to foster a community where all residents have the knowledge, skills, support and confidence to succeed in all areas of life. 

We are the only community health center in the nation also designated as an Area Agency on Aging and an Aging and Disability Resource Center (ADRC) by the State of South Carolina, the Administration on Aging and the Centers for Medicare & Medicaid Services. Called Vantage Point, we are the first place to get accurate, unbiased information on all aspects of life-related to aging or living with a disability.

Pickens County Meals on Wheels (PCMOW) exists to combat the effects of hunger, loneliness and isolation on senior citizens living in Pickens County, and to provide a lifeline through the use of caring volunteers. Our vision is to provide a continuum of care for active, aging and homebound individuals by providing daily meals, social, cultural and educational activities and connecting individuals and their families to the services to ensure they are a valued member of our community. Pickens County Meals on Wheels serves hot, weekday meals to over 360 homebound clients in Pickens County daily. The program also includes Young at Heart, a place for older adults to participate in activities, social connection, and dine for lunch. No cost and low cost services are available.

Pickens County Meals on Wheels delivers hot nutritious meals to homebound citizens within Pickens County. We operate three Young at Heart dinning and activity senior centers located in Liberty, Central and Easley. At these centers we offer certified senior group exercise classes, planned activities, field trips and dining. 

Piedmont Agency on Aging provides free home-delivered meals to seniors 60 and older in Greenwood, Abbeville, Laurens and Saluda Counties. Other services provided are senior center meals and transportation to the centers and medical appointments. Low and no cost services are available. 

 Our residents have access to a choice of home health agencies that provide regular personal care assessments. Staff members who are on call 24 hours a day work with residents to tailor plans for all levels of care needed, with services including medication reminders and administration, incontinence care, special nutritional counseling and escort and transfer assistance available if and when needed. Residents also can obtain bathing, dressing and grooming assistance and medication administration as necessary.

Physical, occupational and speech therapies are available on site as conveniences to our residents, who also have access to psychotherapy services. Wellness, walking, balance and exercise classes also are offered on a regular basis.
