The South Carolina Developmental Disabilities Council is a federally funded program that provides leadership in planning, funding, and implementing initiatives that lead to improved quality of life for people with developmental disabilities and their families through advocacy, capacity building, and systemic change.

Home accessibility company specializing in helping seniors age in place by making homes handicap-accessible. Primary products include wheelchair ramps and stairlifts. Flexible pricing options available (rental and purchase). 

Free program for drivers where they remain in their vehicles & trained technicians go through 12 safety checks to make sure they fit in their vehicles & are safe to drive.

We check all lights, mirror adjustments, seat belts, head restraints, seat location as far as seeing over the dash & ability to reach petals. A trained Occupational Therapist will be there most of the time to help anyone that has problems getting in or out of their vehicles, etc.

Dementia Dialogues® is a nationally registered and evidence-informed, intervention program. This 5-module training course is designed to educate community members and caregivers (formal or informal) for persons who exhibit signs and symptoms of Alzheimer’s disease and related dementias (ADRD). The modules cover valuable information and recommendations including an overview of dementia, strategies for effective communication, understanding the environment and ways to promote independence, addressing challenging behaviors, and creative problem solving. Dementia Dialogues® is offered nationwide by Certified Instructors in-person and virtually, and as a virtual self-guided training at no cost to participants. Dementia Dialogues® is provided through Office of the Study of Aging at the Arnold School of Public Health, University of South Carolina.

PurFoods, LLC dba Mom’s Meals (“Mom’s Meals”) is a USDA-inspected food preparation and home-delivered meal service dedicated to providing delicious meals to our customers nationwide. Mom’s Meals strives to be the leading provider of aging, disabled and patient nutrition solutions that preserve health, help combat chronic disease, support recuperation and nourish independence for high-quality living at home. Mom's Meal's mission is to help keep seniors & individuals with disabilities in the comfort of their homes as long as possible by delivering nutritious meals directly to their homes. We have grown to provide daily in-home nutrition support for over 180,000 unique clients across the country. We utilize a HIPAA-compliant database that tracks each client's ordering history, feedback, notes from Case Managers, etc. Every client's order is maintained in their respective account and includes the number of meals for each delivery and delivery verification. 

Professional Case Management was founded with the goal of providing the best possible care in the best possible place – the home. PCM caregivers follow the core principles of FOCUS, EXPERIENCE, QUALITY and COMPASSION to provide in-home care services with the highest standards and ethics in the industry. PCM’s dedicated and knowledgeable professionals deliver a full range of services from monthly visits or help with daily activities to extensive, round-the-clock continuous care. In the comfort of home, clients and families are able to maintain the independence and quality of life they have earned.

The South Carolina Assistive Technology Program (SCATP) is a federally funded program concerned with getting technology into the hands of people with disabilities so that they might live, work, learn and be a more independent part of the community. Our goal is to enhance independence, productivity and quality of life for all South Carolinians through access to assistive technology devices and services.

Our Mother's Voice provides information to empower individuals and families to advocate for appropriate and high-quality care and services when they need long-term care for themselves or loved ones.  We focus primarily on Quality of Life, Quality of Care, Resident Rights, and how to advocate within the service delivery system and the regulatory system.  We provide online information and in-person presentations for a wide variety of audiences.

We are the only full placement service that can assist with Medicaid placements for assisted living and skilled care!

Full Service Placement Assistance - Whether you are thinking about assisted living, dementia care, independent senior living or skilled nursing, we can help you identify the right level of care and find the best community that fits the needs of your loved one.  This is a no cost service to you.  It's about what matters most to you and your family, current and future care, personal preferences, location and budget.  We arrange the visits at your chosen communities, personally accompany you on tours, and coach you to ask the right questions.  We track the communities compliance with state regulations.  You don't have to do this alone!

Elder Care Consulting - There is so much going on.  You are trying to balance family, work, and supporting a loved one.  We can assist with cutting through the never-ending options, anecdotes, and advice.  We help you sort through all of it, create a realistic step-by-step plan to navigate the many decisions you will face, and help you access appropriate, vetted resources.  Bringing in home care, putting a plan of care in place so you aren't dealing in a crisis.  Need someone to be your loved ones care advocate as the adult children are out of town or out of state?  We can be that extension and extra hands and eyes for your loved one while they are in the hospital or rehab.

Palmetto Medicaid Eligibility Consulting, LLC, (PalmettoMEC) specializes in helping South Carolina residents qualify for nursing home and in-home care Medicaid. As experienced consultants and planners, we partner with the applicant or a representative in the application process to obtain all documents needed to complete the nursing home application and meet Medicaid program requirements; we then organize the application and promptly submit it to the Medicaid agency, the SC Department of Health and Human Services (SCDHHS) for review and approval. PalmettoMEC's consultants have more than 30 years of Medicaid eligibility, policy, and case–management experience administering long-term care programs.

VR prepares and assists eligible South Carolinians with disabilities to achieve and maintain competitive employment.

We serve individuals with spinal cord injuries to assist them in finding resources for their needs, questions, and concerns.

We provide technology services, educational classes and programs for senior adults and their families, so they can be better connected. Bill has worked with 100’s of seniors throughout South Carolina teaching them how to use their technology more confidently, effectively and safely. Diversified Training Consultants Group provides senior care management and dementia care planning while helping families navigate long term care.

Memory Fit provides Memory Training as a prevention for the development of dementia.  Our Memory Training courses are 4 weeks each month for a total of 12 weeks.  Memory Foundation is week one in which participants learn about the brain.  Memory Everyday starts week two through week 10 in which participants learn the types of memory we use daily, strategies to navigate real life everyday memory issues, the Big 4 Factors for the prevention of dementia, and food that support brain and heart health.  Participants are encouraged to attend and participate all 12 weeks to receive the fullest benefit from our Memory Training courses.  All participants receive a complimentary memory assessment.  Please visit our website at memoryfit1.com to learn more.  You may also contact a Certified Memory Trainer at 803-205-4006 for additional information or questions.  This program is ideal for individuals with normal age-related memory complaints.  Our courses is not for people diagnosed with dementia or Alzheimer's Disease. The Montgomery Speech, Language, and Hearing Clinic provides evaluation and treatment services for all ages. Focus areas include articulation, language, voice, hearing, stuttering, and everyday communication, among others. Individual and/or group programs may be recommended. Innovative services that align with research and best practices are conducted by graduate students under the direction of certified and licensed speech-language pathologists and audiologists. Individuals receiving services at the Center may be invited to participate in research during their treatment.
